Before your first visit we do require some information about you and your health.

• You may download and print paper copies of the new patient registration and medical history forms and complete them. You can either bring the completed forms with you on your visit or mail to us ahead of time.

​

• You may use the traditional option of completing the registration and medical history information in our office on the day of your visit. If you choose this option please plan to arrive 15 minutes early to allow sufficient time for this.
